SUSPENSION TRANSFER FORM

Dear Student,

Please fill out the form below in it’s entirety. Please type a short explanation of the circumstances that prohibited you from being a successful student at the last college/university, and what you will do differently here at North Central Texas College that will allow you to be successful with us. You will be placed on probation your first semester at NCTC and the following stipulations will apply.

• Limit enrollment to 12 semester hours

• Maintain a “C” average in all course work

• If a 2.0 GPA is not earned, you will be placed on Academic Suspension
