Please provide some or all of the following information.  

• A diagnosis with terminology consistent with the DSM-IV.   Multi-axial diagnosis preferred with current GAF.

• A list of the diagnostic tools used to reach the diagnosis.  

• A summary indicating the onset, longevity, and severity of symptoms.

• A report of how the current diagnosis will impact learning and other major life activities.

• A list of medications, treatment, or services currently prescribed.

• Suggestions or recommendations for accommodations and support services.

Please note that all documentation should be reasonably current.   Further documentation may be requested if the documentation does not support the need for a requested accommodation.   Also, all supporting documentation submitted must be from a qualified professional.  

Upon receipt of documentation, the ACCESS Committee will determine on a case-by-case basis what accommodations are both reasonable and appropriate.
