Please provide some or
all of the following information.
A medical
diagnosis of the head injury or traumatic brain injury.
A list of
the diagnostic tools used to determine cognitive ability, including
standardized scores or percentiles.
Information
regarding the impact of medication on the student's ability to function
in a post-secondary environment.
A report
on how the injury will impact learning and other major life activities.
Suggestions
or recommendations for accommodations, assistive devices, or support
services.
Please note that all documentation
should be reasonably current. Further documentation may be
requested if the documentation does not support the need for a requested
accommodation. Also, all supporting documentation submitted
must be from a qualified professional .
Upon receipt of documentation,
the ACCESS Department Committee will determine on a case-by-case
basis what accommodations will be reasonable and appropriate.
