Please provide some or all of the following information.

• The medical diagnosis of the physical disability or systemic illness.

• A summary of the diagnostic tools used, including evaluation results.

• A list of present symptoms.

• Information regarding the impact of medication on the student's ability to function in a post-secondary environment.

• A report on how the disability will impact learning and other major life activities.

• Suggestions or recommendations for accommodations, assistive devices, or support services.  

Please note that all documentation should be reasonably current.   Further documentation may be requested if the documentation does not support the need for a requested accommodation.   Also, the supporting documentation submitted must be from a qualified professional .

Upon receipt of documentation, the ACCESS Department Committee will determine on a case-by-case basis what accommodations will be both reasonable and appropriate.
