Suggested Documentation of

Learning and Language Disability

 

Please provide some or all of the following information:

  • A diagnosis with terminology consistent with the DSM-IV.   Multi-axial diagnosis preferred with current GAF.
  • The date of the original diagnosis and the date of the current evaluation.
  • The diagnostic tests used and a description of the diagnostic criteria.
  • A report indicating test scores/data.
  • A statement indicating the functional limitations of the disability on learning and/or on other major life activities.
  • Any recommendations for accommodations, adaptive devices, assistive services, and support services.
  • Information concerning current treatment, medications, assistive devices or services currently prescribed.
  • A statement of the expected progression or stability of the disability over time.  

 

Please note that all documentation should be reasonably current.   Further documentation may be requested if the documentation does not support the need for a requested accommodation.   Also, all supporting documentation submitted must be from a qualified professional .

 

Upon receipt of documentation, the ACCESS Department Committeewill determine on a case-by-case basis what accommodations will be both reasonable and appropriate.
