Please provide some or all of the following information.

• A Certificate of Deafness from the Texas Commission for the Deaf and Hard of  Hearing.

• An audiogram by a licensed audiologist.

• Information about functional limitations as a direct result of hearing loss.

• Information regarding the use of hearing aids.

• Any additional medical information, as appropriate.

Please note that all documentation should be reasonably current.   Further documentation may be requested if the documentation does not support the need for a requested accommodation.   Also, all supporting documentation submitted must be from a qualified professional .

Upon receipt of documentation, the ACCESS Department Committee will determine on a case-by-case basis what accommodations will be both reasonable and appropriate.
