Suggested Documentation of

Attention Deficit/Hyperactivity Disorder

 

Please provide some or all of the following information:

•  A diagnosis with terminology consistent with the DSM-IV.   Multi-axial diagnosis preferred with current GAF.  

•  A clinical history of the disorder, including the date of the original diagnosis.

•  A list of present symptoms.

•  A list of the diagnostic tools/instruments used to reach the diagnosis.

•  A report indicating tests scores/data.  

•  A list of medications, treatment, or services currently prescribed.

•  A report of how the current diagnosis will impact learning and other major life activities.

•  Suggestions or recommendations for accommodations, assistive devices, and support services.

•  A statement concerning the expected progression or stability of the disability over time.

 

Please note that all documentation should be reasonably current.   Further documentations may be requested if the documentation submitted does not support the need for a requested accommodation.   Also, all supporting documentation submitted must be from a qualified professional .  

Upon receipt of documentation, the ACCESS Department Committee will determine on a case-by-case basis what accommodations will be both reasonable and appropriate.
