How to Apply for the MATRIX Grant


Applications:

MATRIX APPLICATION for non-credit students.

 

MATRIX APPLICATION for incumbent workers.

Make sure to attach:

Copy of driver's license or any other valid form of identification

Proof of unemployment or underemployment

Copy of HS Diploma, GED Certificate, or college transcript

Bring proof of immunizations (CNA, CMA, Phlebotomy, Surgical Tech)

Complete an enrollment form
